Special Education Forms

Day and Per Session Home Instruction are requird to complete IEP  & Transitions forms by the requested date.  Assistance is available through Home Instruction Coaches and through borough point people. 

 IEP requests will either come from Home Instruction Guidance Counselors or from the School Of Affiliation, no one else. 

 

Please do not communicate goals, or any other IEP related information to anyone outside HI until your forms are submitted and approved. 

 

Contact your borough  Guidance Counselor with any concerns or questions.  

Transition 

Transition is a results-oriented process that is focused on improving the academic and functional achievement of students with disabilities to facilitate the student’s movement from school to post-school activities with the goal of continued education, employment, and/or independent living.

 

 Transition documents are found below with detailed information about their applicability.   A Home Instruction guidance counselor will request specific forms when needed. 

SMART GOAL WRITING

The IEP Goals bank is a great resource to aid in writing SMART goals. Please consult this bank when you are writing goals for your students. 

 

Use this Smart Goal Template to draft your goals, before you begin your online SEISIS goal form.

Progress Reporting and Sesis Goal Update

Progress reports will be requested, however teachers are required to choose and complete the correct level progress report which includes the SESIS smart goals suggested for inclusion in the final IEP. 

Please use the IEP Writing Suggestions for information on what is expected to be included in each section of the report. If submissions are not complete or do not follow the approved formats they will not be approved.

A minimum of 2 SMART ANNUAL GOALS are required for teachers not covering all subjects. 

We prefer 4 goals submitted for each update. 

 

Please ask for assistance, forms completed inadequately will not be approved and will need to be re-completed.

504 Plans 

Completed 504 Accommodation Forms must be submitted to the student’s affiliated school for review and approval.  Please obtain a copy from the school, once they have completed their review process.  Please give a copy to your supervisor.  If any questions arise contact our Guidance Department.

Transition Documents

Vocational assessments are used for all students with an IEP ages 12 and up and are alays completed annually. 

 

Vocational assessments MUST be completed and the results  are rquired to be incorporated into the Transition sections of the student's IEP for all student who are 15 and up.

 

There are two vocational assessments available: one for students who are able to read and one for non readers. 

 

The vocational assessments for readers includes a section for parents to complete, as well as teachers. When completing a non readers vocational assessment ensure you as the teacher and parent complete the required sections. 

There is also a Vocational Assessment CDOS,  this will only accompany a Vocational Assessment for an IEP student who is approching their CDOS credential.  

 

Keep in mind, the assessments are interview / inventories; therefore IT IS appropriate for you to write student and parent answers if it is a more appropriate for completion. 

Exit Summaries

Student Exit Summaries should be completed during the final year of high school or the year which the student ages out of NYC DOE services.  The summary must be  provided to the student, and when legally appropraite the parent prior to school exit. 

 

Student with IEP's with standard criteria will coimplete the Standard Exit Summary. 

 

Aleternate Assessment students will complete the NYSAA Exit Summary. 

 

Lastly, students who are exiting with a CDOS credential will complete the Standard Exit and CDOS Exit summary. 

 

Parents / Guardians will receive a consultation with a Home Instruction transition team member to help explain the overall transition process and what services are available as post secondary options.
