In order to request Home Instruction Services the parent/guardian will need to notify the school’s guidance counselor and work with the school of affiliation to submit the following documents (High school students also have to submit permanent record and transcript)

 

NOTE: Social Security Number is not required for HIPPA

What is

Home Instruction?

Home Instruction Schools provides educational services for all school aged children (grades K-12) who are unable to attend school due to orthopedic, non-orthopedic/medical, or psychiatric reasons.

 

Home Instruction Schools services students in all five boroughs of NYC. Home Instruction is an interim, non-diploma granting program. Home Instruction provides academic services to limit the educational effects of a long-term absence (four weeks or more).

 

 Home Instruction operates during day and after-school hours. 

How do I apply for Home Instruction services?

On the Notice of Potential Disclosure of Student Records and Opt-Out Form page you will find the full Notice and Opt-Out form.  Both documents are available in all 10 of the DOE-supported languages. 

NOTE: Home instruction is not available for students who cannot attend school because they have not met immunization requirements.  Please contact the Office of Home Schooling.
