In this page, you'll find everything you need with respect to IEPs, Transitions, and goal writing.

Exit Summaries

Student Exit Summaries should be completed during the student's final year of high school or the year in which the student ages out of NYC DOE services. The summary must be provided to the student, and when legally appropriate, also the parent or guardian prior to school exit.

 

Students with IEPs with standard criteria will complete the Standard Exit Summary.

 

​Alternate Assessment students will complete the NYSAA Exit Summary.

 

Students who are exiting with CDOS credential will complete the Standard Exit and CDOS Exit Summary.

 

​Parents and guardians will receive a consultation with a Home Instruction transition team member to help explain the overall transition process and what services are available as post secondary options.

IEP Point People

The IEP Point People are here to support teachers with IEP assessments and curriculum needs. This includes, but is not limited to, SANDI, DLM, data collection, curriculum, and IEP updates. They are available to meet teachers during day and per session hours. Please email them with your concerns.

IEP Requests

IEP requests will either come from Home Instruction guidance counselors or from the school of affiliation, and no one else.

​

Please do not communicate goals or any other IEP related information to anyone outside of Home Instruction unless you speak with us first and until your forms are submitted and approved.

​

Contact your borough guidance counselor with any questions or concerns.

Parent Request for Evaluation

This form is for parents requesting either an initial evaluation or a reevaluation. Please print or email this form to parents in need of evaluations and contact your guidance counselor about next steps.

SMART Goal Writing

The IEP Goals bank is a great resource to aid in writing SMART goals. However, please note that goals must be written with your student in mind and should not be generic. Please consult this bank when you are writing goals for your students. ​

Transition

Transition is a results-oriented process that is focused on improving the academic and functional achievement of students with disabilities to facilitate the student's movement from school to post-school activities with the goal of continued education, employment, and independent living.

Transition documents are found to the left, with detailed information about their applicability. A Home Instruction guidance counselor will request specific forms when needed.

​

Vocational assessments are used for all students with an IEP ages 12 and up and are always completed annually. Vocational assessments must be completed, and the results are required to be incorporated into the Transition sections of the student's IEP for all students aged 15 and above.

​

There are two vocational assessments available. One is for students who are able to read, and one is for non-readers. The vocational assessment for readers includes a section for parents and teachers to complete. When completing a non-reader vocational assessment, ensure that you as the teacher and the parent or guardian complete the required sections.

There is also a Vocational Assessment CDOS. This will only accompany a Vocational Assessment for a student with an IEP who is approaching their CDOS credentials.

​

Keep in mind, the assessments are interviews and inventories. Therefore, it may be more appropriate for you to write the student answers and parent answers.  â€‹
