This is the page you go to for any student-related emergency situations. All Home Instruction employees, including after school employees, are mandated reporters. Mandated reporters have frequent contact with children and have an early opportunity to help get them the intervention, support, and services they need to stay safe and well. It is the responsibility of the Home Instruction teacher to report any untoward incidents such as suicidal ideation, alleged child abuse or neglect, threats of planned violence, etc.
What to do
In all emergencies, including those not covered here, ALWAYS CALL 911.

Always report suicidal ideation or a missing child.

Contact our Guidance Department at (718) 794-7200 and immediately contact your AP and Per Session Coordinator.

Always call the Guidance Department and speak with a representative. Do not only leave a message with any reporting concerns or questions. Emergency situations can be overwhelming, but we will walk you through the process.
Child Abuse Protocols and Procedures
If you as a Home Instruction teacher suspect that your student (17 or under) is a victim of child abuse or neglect, as a mandated reporter, you must call the State Central Registry (SCR) immediately at 1-800-635-1522 and report the suspicion of child abuse.

If the SCR accepts your report, you must:

Record the name of the person at SCR who took the report

Record the State Registry Call ID Number

Complete the report of Suspected Child Abuse or Maltreatment Form (LDSS-2221-A)

Complete the Home Instruction Incident Form

Call the Home Instruction Guidance office at (718) 794-7200, ext. 1, option 2, to report the incident.

Email the Home Instruction Incident Form to the Guidance office.

​If the SCR does not accept your report as the teacher, you must:​​

Record the name of the person at SCR who did not accept the case.

Complete the incident form and forward to the Home Instruction Guidance office.

Call the Guidance office or notify your Assistant Principal that the report was not accepted.

​For students 18 and older, as the Home Instruction teacher, you must call Protective Services for Adults and Disabled (APS) at (212) 630-7853. If the case is accepted, request a reference number. Provide your telephone number so that the assigned case worker can make contact. Make sure you provide the Home Instruction guidance counselor with the case worker’s name and contact information so that the Home Instruction guidance counselor can follow up.

Missing Child Protocols and Procedures
For teachers instructing in the home
​If the student leaves the home during instruction without permission, inform the parent, guardian, or chaperone immediately. Do not leave the home until you have contacted the HI Guidance office (718) 794-7200, ext. 1, option 2. Do not just leave a voice message. You must speak with someone in the Guidance office.

If parent, guardian, or chaperone informs you that the student is missing and their whereabouts are unknown, you must contact the HI Guidance office immediately with the chaperone present. Do not just leave a message. You must speak with someone in the Guidance office.

For teachers instructing outside the home
​If the student leaves the lesson without permission, call 911 immediately. Once the police arrive, get pertinent information: The officer's name, badge number, and precinct number.

Then call the parent, guardian, or emergency contact and the HI Guidance office.

If your student does not show up for instruction, contact the parent, guardian, or emergency contact immediately. If they know why the student is not available, then this is an absence.

If the student's whereabouts are unknown, you must contact the HI Guidance office immediately with the chaperone present. Do not just leave a message. You must speak with someone in the Guidance office.​​​​

Suicidal Ideation protocols and Procedures
Whether teaching in the home, outside the home, or online, do not leave the child alone. Do not leave without reporting suicidal ideation to the parent, guardian, or chaperone. Call the Home Instruction Guidance office and your supervisor with the chaperone present. Do not just leave a voice message. You must speak with someone. If you are teaching outside of the home, do not leave the child or location until the parent, guardian, or emergency contact picks up the child.

If the child is in imminent danger to themselves
Call 911 if the chaperone will not. Once EMS or police arrive, get pertinent contact information such as ID or Police badge number, name, precinct number, precinct phone number, and the name of the hospital where the child will be taken. Then call the Home Instruction Guidance office at (718) 794-7200, ext. 1, option 2, with the chaperone present. Please do not just leave a voice message. Speak with someone directly. Contact your supervisor or per session coordinator to inform them of the incident and that you have already reported the incident to the Guidance office. Finally, submit an anecdotal to the Home Instruction Guidance office immediately.

If the child is expressing suicidal ideation
Inform the parent of what the child said and that, as a mandated reporter, it must be reported. Call the Home Instruction Guidance office at (718) 794-7200, ext. 1, option 2 with the parent present. Do not just leave a voice message. Speak with someone directly. Contact your supervisor or per session coordinator to inform them of the incident and that you have already reported the incident to the Guidance office. Finally, submit an anecdotal to the Home Instruction Guidance office immediately.
